Table of contents - Section 1, ensure that for Q1 to 11, there is one and only one box ticked in every row.

- Section 2 Question 1, if the total is not equal to the sum of Question 4 then the source and year (in question 2 and 3) should differ with the source and year (in question 5 and 6).

- Section 2 Question 1, if no data is available then put number 99999. Then put code 9 at Question 2 and code 9999 for year at Question 3.

- Section 2 Question 4, if no data is available then put number 9999. Then put code 9 at Question 5 and code 9999 for year at Question 6.

- Section 2 Question 7, if no data is available then put number 9999. Then put code 9 at Question 8 and code 9999 for year at Question 9.

- Section 2 Question 10, if no data is available then put number 9999.

- In sections 4.2, 4.3, 4.4, 4.5, 4.6: for column number 1, if the response is code 1 (Hapana) and column number 2 is not circled then circle ‘H’ and state – hakutaja.
